About The Role As a Clinical Care Manager at our Traralgon home you will be responsible for bringing The Bupa Promise to life for our residents: know me and my needs, help steer my decisions, be there when I need you. Your genuine passion for helping others and making a difference to our resident’s quality of life will be evident in everything you do, as will your ability to empathise and put yourself in their shoes. You will lead by example to set the standard of delivery of Bupa’s person-centred care to our residents, ensuring the highest standard of ongoing assessment, care planning, evaluation and clinical governance. Key accountabilities will include:

  • Ensuring the delivery of quality clinical and person-first assessments for all residents
  • Overseeing the coordination of ACFI in collaboration with the GM
  • Delivery of the highest quality clinical care
  • Management of medication in accordance with resident needs and legislative requirements
  • Lead the care home team in the delivery of person-centred care, including coaching, mentoring , staff allocations and delegation, performance management and education
  • Management of person-centred care planning and evaluation for residents

In addition, you will have:
  • Bachelor of Nursing or Equivalent is essential, with a post grad qualification in aged care highly regarded
  • Current AHPRA is essential
  • Proven track record in delivering quality clinical care within an aged care or acute setting
  • Excellent clinical assessment skills
  • Care planning capabilities
  • Highly effective communication, leadership and relationship management skills
